Liability Release

I understand that participating in Elite4Life OCR Training activities, involves a certain degree of risk that could result in injury or death. In consideration of the benefits to be derived, after carefully considering the risks involved, I do hereby waive, release, absolve, forever discharge, and do further agree to indemnify and hold Elite4Life OCR Training, its employees, trustees, officers, volunteers, affiliate, and agents from any and all costs and expenses, including but not limited to: any and all medical/hospitalization costs, attorneys’ fees, reasonable investigative and discovery costs, and/or court costs.

• I assume all liability for any and all personal injury, bodily injury, illness, and property damage that result in participation in such activities. 

• I agree that I will not bring any lawsuits nor make any demands nor pursue any complaints against Elite4Life OCR Training as a result of my participation in activities. 

• I agree that participation in this activity is voluntary and I have read and understand the inherent risks involved in this activity.

• I understand that these risks exist despite the safety precautions and procedures implemented by Elite4Life OCR Training.

• I agree to obey all rules and policies mandated by Elite4Life personnel.

Photo, Media, & Copyright Release

I grant Elite4Life OCR Training my permission to photograph, videotape, and/or audiotape me while taking part in Elite4Life OCR Training activities. These photographs/videos/audiotape will remain the property of Elite4Life OCR Training and may be used in advertising and/or marketing campaigns on Elite4Life OCR Training’s website, social media pages, and for promotional and informational material, including but not limited to: social media, flyers, brochures, newsletters, emails, advertisements, newspaper articles, TV or cable interviews/promotions. I hereby waive and release any rights to compensation for, or ownership of, such images and/or sounds.
